•  Home and Community Care services (HACC) can provide basic support services so that frail older people and people with a disability can continue to live in their community. These services provide eligible people with help at home, support in getting out into the community, and a break for carers.  
As clients needs increase, additional services are required to enable clients to remain living at home. integratedliving is able to offer in many areas, a range of aged care packages:

•  Community Aged Care Packages (CACP) are flexible and designed to help with individual care needs as additional services are required to enable clients to remain living at home.

•  Extended Aged Care Home Packages (EACH) are available for people who require a high level of care and provide an individual planned and coordinated service.

•  Extended Aged Care Home Packages Dementia (EACHD) provide the full range of services that EACH offers however, strategies are designed to meet the specific needs of care recipients with dementia who experience challenging behaviour.

These packages provide the supplementary supports for frail aged persons to continue living in their own homes.
