This is an eight-week program, especially designed to help you regain or maintain your wellbeing and independence.

Short-term Restorative Care program

integratedliving recognises the importance of seniors to being able to live independently and safely at home. Our Short-Term Restorative Care program (STRC) seeks to ensure that people’s health goals are met to improve their wellbeing.

When you join the STRC program, you will collaborate with our wonderful team of accredited health specialists to identify your goals and tailor therapy based on your General Practitioner’s advice and your personal needs. Receive restorative care support from the comfort of your home or you may also choose to come in to one of our Wellness Centres.

Services available under this program

We provide access to a variety of services that support older Australians to live independently in their homes. They include;

  • Nursing
  • Occupational therapy
  • Physiotherapy
  • Audiology
  • Speech pathology
  • Optometry
  • Podiatry
  • Psychology, social work or counselling support 
  • Dietitian and nutritionist services
  • In home help including personal care
  • Home upkeep services including gardening, lawn and home maintenance
  • Exercise programs.

The Short-Term Restorative Care program is currently available in the below regions:

  • Hunter and Illawarra region, New South Wales
  • Far North, Northern and the Wide Bay region of Queensland
  • East Arnhem region, Northern Territory
  • The Gippsland region, Victoria
  • Australian Capital Territory
  • Southern Tasmania.

Eligibility criteria*

  • You need to be aged over 65 or over 50 for Aboriginal & Torres Strait Islanders.
  • Some services may be available to people aged under 65.

Our team of trained health professionals and coaches will work with you to achieve and maintain your health goals.

Yes, this service can be aligned with your values and preferences. We will take the time to learn your individual cultural and care needs so they are always considered when making their recommendations. They will also encourage you to set meaningful goals and help you to achieve them. You may also have your preferred support person included during the consultations or access interpreting services if required.

You may not be eligible for this program if you were hospitalised with overnight stays in the last three months. Contact us on 1300 782 896 to find out more.

Your preferred Allied Health professional will need to be registered to our supplier framework before they are allowed to work with you to deliver services under our Short-Term Restorative Care (STRC) program.

Yes, you can take up to seven days leave from the Short-Term Restorative Care (STRC) program. If you are absent, or is hospitalised for more than seven consecutive overnight stays, the STRC program will automatically stop and you will need to reapply to restart the program. The program has to be completed in one block of an eight-week period.

Depending on your health goal, an Occupational Therapist can assess you and your home. They may prescribe new equipment like mobility aids, personal alarms, hand rails, kitchen aids and more. Equipment can be purchased through the program and remains yours once the program ends.

Yes, your Short-Term Restorative Care (STRC) program approval lasts six months from the date of assessment. A new request for a STRC program would need to be submitted after this period. Please note that integratedliving has a waiting list for this program in certain regions.

One of our health professionals will contact you and set you up to begin the program. They will also contact your general practitioner and work closely with you to design an individualised eight-week program, completed under their guidance. A review will be conducted midway and towards the end of the program to ensure that you're getting the most out of the program.

All health services selected must be in line with your program goals. You can work with your designated health coach to map these goals at the start of your program.
