Diabetes Wellness

Manage the high and lows that come with having diabetes plus nutrition and exercise tips.

Benefits

Designed for older people who are living with diabetes or at risk of developing diabetes, the program supports you to recognise the lifestyle factors that can affect your diabetes health and ways you can make changes to stay well and active.

If you are at risk of getting diabetes the program can help you avoid it or if you have diabetes, you can learn how to prevent it from progressing to a more serious form.

While everyone will experience different benefits from the Diabetes Wellness program, you can expect to recognise the early signs of diabetes, how to manage them and how to avoid a diabetic emergency. 

  • Manage your diet, nutrition and blood glucose levels.
  • Stay active while living with diabetes.
  • Understand and manage the subsequent conditions that can develop.

About the program

The Diabetes Wellness program is delivered by our expert clinical and allied health staff who understand everyone’s diabetes journey is different and tailor the program to suit your needs.

When you sign up to our Diabetes Wellness program you will also complete our Foot Wellness program. The combined program provides specialist education about healthy feet and falls prevention linked with education about damage to nerves in the feet related to diabetes.

As a Diabetes Wellness program participant you will receive health assessments prior to starting the program and throughout the program. This enables clinicians to monitor your progress, make any adjustments to your customised program and help celebrate your achievements.

As a result of COVID-19, our Wellness programs have been moved to a virtual setting meaning you can access it from wherever you live. Delivered once a week via a Zoom video call, you’ll be able to speak openly about your concerns and even socialise with new friends, all from the comfort and safety of your own home. One of our Wellness Facilitators can even help you get set up if you’ve never used video calling before and all you need is internet connection.

As part of the programs, you will receive;

  • An information book for you to refer back to
  • Loan of an iPad (for the duration of the program) to help increase your access to support in managing diabetes
  • An individualised health check and health plan.

Eligibility criteria*

  • You need to be aged over 65 or over 50 for Aboriginal & Torres Strait Islanders.
  • Some services may be available to people aged under 65.

How can I pay?

There are several ways to access our services and programs, including the funding options outlined below.

Government Funding

Home Care Package, Commonwealth Home Support Program and Department of Veterans’ Affairs options.
