Our Concierge team manages the full aged‑care onboarding and navigation process, making sure clients move smoothly from entry‑level support to more complex care when their needs change. Our Care Managers guide clients through the Support at Home journey, ensuring each person receives the right mix of services and funding when they need it. As Senior Manager - Care Partnering Amy explains, the program is designed “to be responsive to where people are at and making sure they have the right package of resources available to them to meet their needs at any point in time.”

To deliver this, Care Managers review assessments, track changes in health or confidence and adjust supports so clients remain well and independent. They use existing assessments to avoid clients repeating their story, build budgets that reflect individual priorities and advocate when needs grow.

Working closely with clinicians, families and community supports

Behind the scenes, the team works closely with clinicians, support workers, families and GPs to ensure every detail is understood. Amy describes how Care Managers “translate clinical information into what is essentially human service,” helping clients understand what changes mean for their daily life.
