What is Concierge?

Our Concierge team manages the full aged‑care onboarding and navigation process, making sure clients move smoothly from entry‑level support to more complex care when their needs change. As Concierge Senior Manager Ash explains, “we hold a client’s hand through the aged care journey all the way from entry level support to more complex support.”

A safety net of teamwork behind the scenes

Behind the scenes, Concierge operates like a care‑navigation safety net. Ash describes it as “almost like a friend group for our clients … you pass the parcel”, meaning everyone works together to make sure no client falls through the cracks.

This dedicated teamwork strengthens the client experience by ensuring every person is cared for holistically, not just administratively.

A day in Concierge

A normal day in Concierge team is a blend of advocacy, investigative work and heartfelt connection.

The team starts by checking My Aged Care for any movement in approvals, referrals or funding - a process Ash describes as monitoring “daily… making sure we’re the first who know when you get your package assigned.”

Throughout the day, Concierge calls clients to check in, reassure them and keep them updated on what’s happening behind the scenes. These calls are vital and help clients better understand a system that can be overwhelming.

Small actions that make a big difference

Our Concierge team work to best shape our clients' experience and outcomes. "We want to make sure that you get what you need," Ash said. "And stay at home for longer.” It’s this human touch and reassurance that helps clients understand the advocacy and persistence they have on their side.

Scheduling

Behind every personalised service delivered to our clients is a team quietly orchestrating thousands of moving parts around regional and rural Australia each week. Our Scheduling team plays a crucial role in ensuring our clients receive the right support at the right time, and they do it with remarkable skill, care and collaboration.

Coordinating the right support at the right time

At the core of their work is a finely tuned process that matches team members to clients for every service. Each day, schedulers consider skills and requirements, times, location, preferences, availability and continuity of care.

It’s a balancing act that requires significant attention. Behind every appointment is a thorough scheduler carefully aligning client needs with workforce capacity while adapting to changes and ensuring services run reliably. This work goes a long way to support positive client outcomes, often without drawing any attention to its complexity.

Collaboration that strengthens the client experience

One of the team’s greatest strengths is their culture of collaboration. When unexpected changes arise such as staff illness, urgent client needs or last‑minute disruptions, schedulers step in for one another across regions and teams.

They share knowledge, redistribute workloads and support colleagues who may be navigating a challenging day. 

This teamwork ensures clients continue receiving uninterrupted care, even when the behind‑the‑scenes coordination is immense.

What happens in Scheduling?

A typical day for our Scheduling team is anything but predictable. While planning ahead for future services, the team is simultaneously responding to real‑time changes. These include:

  • Finding replacement team members.
  • Adjusting services.
  • Communicating changes with team members and clients.
  • Solving logistical challenges.

What most people don’t realise is that our schedulers make hundreds of decisions every single day, mostly connected to many others. Each one contributes to consistently high‑quality support for our clients and a smoother experience for our team members.

Our Scheduling team’s commitment to clients sets them apart. They don’t simply fill shifts. They understand the importance of continuity, relationships and personalised care.

Schedulers approach this careful work with empathy, determination and problem‑solving prowess. Their dedication ensures that clients feel supported, valued and confident in the services they receive.

Our schedulers do an amazing job navigating so many variables and the challenges that brings. It's a key role that allows us to deliver services to so many clients and homes across a very broad stretch of regional and rural Australia.

Client care management and Care Partners

Our Care Partners guide clients through their Support at Home journey, ensuring each person receives the right mix of services and funding when they need it. "The Support at Home program is designed to be responsive to where people are at," said Amy, integratedliving's Senior Manager of Care Partnering. "And making sure clients have the right package of resources available to them to meet their needs at any point in time.”

To deliver this, our Care Managers review assessments, track changes in clients' health or confidence, and adjust their supports so clients remain well and independent. They use assessments to assist clients, building budgets that reflect individual priorities and advocate for more support when people's needs grow.

Working closely with clinicians, families and community supports

Behind the scenes, our Care Partners works closely with clinicians, support workers, families and General Practitioners to ensure every detail is understood.

Amy describes how her team “translates clinical information into what is essentially a human service." They ensure clients understand what changes mean for their daily lives. They also draw on the expertise of our allied health and nursing teams to strengthen decision‑making and ensure care is coordinated in the most optimal way.

A typical day for Care Partners involves reviewing assessments, understanding client preferences, shaping care plans and noticing early signs of change. Care Partners collaborate with clinicians and community support services to create solutions that fit each person’s circumstances.

"They bring a human perspective to clinical information, work creatively with community resources and use virtual models when needed."
