When you have more than two staff who will deliver services to integratedliving’s clients, please use this Stat Dec to confirm that they hold the minimum certifications and qualifications required to perform those services. Upload this signed and witnessed Stat Dec when prompted during the registration process rather than documents for each person. Please remember to tick each service you or your staff will provide.
