When you receive Support at Home funding with integratedliving, you’ll be paired with an experienced Care Partner who will support you along the way.
As your main point of contact, our Care Partners (formerly known as Case Managers) take the time to understand what matters most to you and help shape a care plan that fits your life now and as your needs change.
Your Care Partner will help guide your Support at Home journey
Amy, one of integratedliving’s Senior Managers for Support at Home, says a Care Partner can make a real difference.
“Your Care Partner helps make sure your care plan reflects your needs, preferences and the way you want to live,” Amy said. “Just as importantly, they help you stay in control of the decisions shaping your support. We’re here to make the funding work for you and use it where it will have the greatest impact.”
From choosing services that suit your day-to-day life to helping to stretch your budget further, a Care Partner can take some of the guesswork out of the process and help reduce out-of-pocket costs.
If your situation changes, your Care Partner can also speak up on your behalf with My Aged Care and help with reassessment so you can continue to receive the support you need.
Flexible support that adapts as your needs evolve
One of the biggest advantages of the Support at Home program is its flexibility. As your circumstances change, your care plan can be adjusted quickly – whether that means bringing in extra help for a while or scaling things back as you get back on your feet.
Care Partners can also ease day-to-day pressure by coordinating appointments, keeping in touch with healthcare professionals and loved ones, helping with paperwork and guiding you through a complex system.
“They also look out for your overall wellbeing, helping to pick up concerns early and connecting you with the right support before small issues become bigger ones,” Amy said.
“Our Care Partners bring experience, compassion and a genuine commitment to every person they support. Their focus is helping people feel confident, supported and able to continue living independently and safely at home,” Amy said.
