Assistive Technology and Home Modifications (AT-HM)

Make your home safer, easier and more accessible with subsidised support.

integratedliving helps older people approved for Support at Home use their Assistive Technology and Home Modifications (AT-HM) funding. Our team arranges occupational therapy assessments, sources and installs equipment and home changes like grab rails, ramps and bathroom modifications, and shows you how to use them safely. We support clients across regional NSW, Queensland and Tasmania.

What is the Assistive Technology and Home Modifications (AT-HM) scheme?

The Assistive Technology and Home Modifications (AT-HM) scheme provides older people approved for Support at Home funding with access to assistive equipment and home changes. Commonwealth Home Support Program participants use different arrangements. People who moved over from a Home Care Package must use any unspent package funds first.

As part of the Government's Support at Home program, AT-HM is designed to help you:

✓maintain confidence with everyday tasks
✓improve safety and accessibility at home
✓reduce risk of falls or injury
✓remain connected to your community

How does the Assistive Technology and Home Modifications scheme work?

To access Assistive Technology or Home Modifications funding, you'll need an aged care assessment.

You may be eligible if you:

  • require equipment or home changes to remain safe and independent
  • have mobility, sensory, cognitive or functional challenges
  • need support to continue living at home safely
  • have an assessed need for assistive technology or home modifications

Getting started with AT-HM

    • Get an assessment

Complete an aged care assessment through My Aged Care to determine your eligibility.

    • Receive your approval

If you're approved, your funding tier will be outlined in your Notice of Decision and support plan.

    • Get personalised recommendations

Where needed, an occupational therapist or other health professional prescribes the equipment or home changes that suit your needs.

    • Arrange your support

Your provider will source, organise and install the approved equipment or home modifications, and helps you learn to use them safely.

    • Start using your funding

If you live in public or social housing, your state housing provider usually arranges home modifications, so talk to them first.

If your needs change, your provider can request a Support Plan Review to update your equipment or home modification recommendations. You can also request a Support Plan review directly through My Aged Care.
