The Core App is a personal health companion providing an integrated, online health service for individuals and their families who participate in the Core program. The Core App allows you to share health information with your personal care team and family, and monitor and promote health and wellbeing. The App also provides you with easy access to your personalised service wherever you are, helping to keep you living, feeling and being well.

Whats involved?

Speak with your personal health care team and family via phone, video, chat or email. Keep track of your health using interactive assessments and surveys. Join one of integratedliving's Wellness for Independence® Programs, or make friends through a virtual 'Let's have a Conversation' morning tea.

From the comfort of your home or when you're on the go, the Core App allows you to:

  • Interact with your personal health care team to help achieve your health goals
  • Participate in virtual health and wellbeing services
  • View your calendar of integratedliving services and reminders, reschedule or request a new service
  • Communicate with family and friends in a variety of ways Join a virtual community group for activities and conversation
  • Provide feedback on integratedliving services Stay up to date with integratedliving news.

How can I access this service?

You can access this service in several ways, depending on your needs and individual situation.


Am I eligible?

The Core App is only available to integratedliving clients and their family members who participate in the Core program. People who participate in Core will benefit from integrated health and support services focused on prevention, health maintenance and continuity of care.

Yes, you can see what services are available for you and schedule appointments directly. 

Protecting your privacy is important to us. integratedliving is committed to safeguarding the confidentiality of the personal information collected regarding you and your families. Our privacy policy applies to all activities of integratedliving and complies with the Australian Privacy Principles (APPs) under the Privacy Act 1988 and associated amendments. The APPs set out standards and obligations for how we handle people's personal information. This includes how we collect, use, store, and secure personal information, as well as your rights to access or correct your personal information. We also ensure our third-party partners comply with these standards. You can view our Privacy Policy on our website. Yes, you can call our Customer Service Centre on 1300 782 896 and ask to speak to your Core Personal Healthcare Team who will assist you in scheduling new services. 

No. You will have access to assistive technology that will be used to capture your health data (vital signs) on a regular basis. This health data will be stored on your Core App (the technology you use will connect to the Core App and store the data as it's generated) and you and your Personal Healthcare Team will be able to access this data to monitor your health.

No, but your doctor appointments can be recorded in the Core App so that services can be scheduled around appointments with your doctor

Yes, our team will assist you with setting up your individual profile on the Core App and provide you with ongoing support to ensure it is working correctly. You will also receive a detailed, easy to follow guide to using the Core App when you become a Core client.

Yes, you can use your own mobile or tablet device. Please note the Core App is compatible with the following – Apple iOS 11.0 or later (98.4 MB) and Android OS 5.0 and above (18.67 MB). You can follow the instructions found in the Core App User Guide on how to download the Core App to your device or we can assist you.

You may be eligible to purchase a compatible device through your package funds. Call us at 1300 782 896 and ask to speak to your Core Personal Healthcare Team who will be able to assist you with this request.

If you don’t have a smartphone, the Core App can also be accessed via an iPad or tablet device. If you don't have either of these devices, you can access the Core program by nominating a family member or carer who is able to access the Core App on their smartphone or iPad/tablet. This will enable them to connect with our health and aged care ecosystem to arrange services for you and keep in touch with your designated Personal Health Partner.
