If you live in rural or regional areas, or find it difficult to regularly visit a doctor, our remote health monitoring service makes healthcare more accessible, with support from registered nurses through telehealth nursing, giving you a peace of mind while staying at home.

The process is straightforward to set up. After consultation with your General Practitioner (GP), your needs will be assessed and we will assist you with downloading the applications and getting you started. Our friendly team will then coach you on reading your vital signs and show you how to use the device to send information to our Registered Nurse.  

The information will be stored and monitored, and we will reach out to you and your doctor if we notice any irregularities or changes to your vitals. This proactive approach means you can take action early and avoid unnecessary hospital visits, helping you feel confident in managing your health at home.

What health signs are monitored through our telehealth services?

Our experienced registered nurses will monitor and record the following health signs, securely sent by you electronically:

  • Blood pressure – Monitor your blood pressure levels regularly to help you maintain safe and stable readings.
  • Blood sugar readings – Keep an eye on your glucose levels, with support to manage day-to-day fluctuations.
  • Heart rate – Record your heart rate to ensure it stays within a healthy range, with nurses available to follow up if needed.
  • Symptoms of other chronic conditions – Keep track of general symptoms or changes you experience that may relate to ongoing health concerns, providing you and your care team extra support.

Eligibility criteria*

  • You need to be aged over 65 or over 50 for Aboriginal & Torres Strait Islanders.
  • Some services may be available to people aged under 65.
